You can obtain a certificate of (no) criminal record along with a multilingual form online at https://ekrk.ms.gov.pl/ or https://ems.ms.gov.pl/. To obtain it online, you need a Trusted Profile (Profil Zaufany). The current cost for issuing this information online is 37 PLN (20 PLN for the certificate + 17 PLN for the multilingual form). This fee may change and is independent of Camp America. Requesting the certificate on the website allows you to receive it in English (with the multilingual standard form as an attachment to the certificate from the National Criminal Register), which is a program requirement. The certificate cannot be issued earlier than July 15, 2024. Once you receive the certificate, add it to your application as soon as the option becomes available. If you do not have this option, email the certificate to the Camp America office in Warsaw at [email protected].

You can also obtain the certificate of (no) criminal record with the multilingual form at the Information Points of the National Criminal Register at local district courts. The cost for such a certificate issued by a local Information Point is 47 PLN (30 PLN for the certificate + 17 PLN for the multilingual form). There is no need to have the certificate translated by a sworn translator.

Important! If you do not have Polish citizenship, you will receive the certificate without the multilingual attachment, only in Polish. Translation is not required.
